We think Mom has reached the point where she needs long-term care, and we’re becoming concerned about how long she can safely remain at home. Do we start calling long-term care homes to see who has space?
This is a question families often don’t think about until the need becomes urgent — and Ontario’s long-term care system can be confusing when you’re encountering it for the first time.
The most important thing to know is that you don’t apply to individual long-term care homes.
Your starting point is Ontario Health atHome.
A care coordinator will work with you and your family, assess your loved one’s needs and determine which homes you would like to apply to — generally up to five. Ontario Health atHome will then coordinate the application and placement process.
That doesn’t mean you shouldn’t call or visit individual homes.
In fact, this is where doing some homework early can really help. Tour the homes you’re considering. Ask about daily life, activities, meals, specialized care and what residents can bring with them. Consider something equally important: Will family and friends be able to visit regularly?
Wait times can vary significantly between homes and according to individual circumstances and care needs. If your loved one’s health, safety or care needs change while you’re waiting, contact your Ontario Health atHome care coordinator. Be open about what’s happening so they have an accurate understanding of your family’s situation.
Don’t wait for a crisis to begin asking questions. Navigating unfamiliar waters is much harder when you’re already in one.
Learn about the process. Talk with your family. Tour some homes. Stay connected with your care coordinator.
